OIM/AD can't create user

We have a new install of 11.1.2.3 and following the documentation we've got to 5.7.7 - verifying LDAP Synchronization.  When we try to create the user, in the oim_server1-diagnostic.log file we see :

[LDAP: error code 64 - 00002099: NameErr: DSID-03051133, problem 2005 (NAMING_VIOLATION), data 0, best match of: 'cn=Reserve,<our DN>'

We are connecting to our AD running Win 2012R2 and we're not using OVD.  We have verified that the cn for "Reserve" was created, we also get a similar error when using the Diagnostic Dashboard and running the "Diagnose LDAP reserve container" test.  The "Diagnose LDAP Connection" test runs fine. 

One question, we patched to BP5 but when looking at the "About" page in the Identity System Admin or the top of Diagnostic Dashboard, it still shows 11.1.2.3.0.0 not 11.1.2.3.5 that I would expect to see.  Is there any way to verify that we installed the BP5 patch bundle correctly??
