OID Setup Diagnostics Test failing

I am in the process of implementing SSO / OID with 11510 Instance, But post implementation step is SSO / OID Setup Diagnostics test ,SSO Test went fine but OID Setup Test is failing with Following error messge

ORA-06508: PL/SQL: could not find program unit being called occured in init
Password not registered with Workflow Global Preferences
Identity Management Realm could not be found
Application container is not created properly
ORA-31204: DBMS_LDAP: PL/SQL - Invalid LDAP Session. occured in
svcContainerTest

Please help me to come out to resolve this issue.
