OAM - Authentication Failure URL not working

Hi,
I need help with very basic stuff in OAM 10.1.4.3. I have a login web server (apache with webgate), configured auth scheme as described and successfully protected a couple of my apps with policies. However if i type wrong password/user id I am not redirected to the URL I specified as "authentication failure" action in my policies. It is supposed to be a URL on the same login web server, i.e. /login_failed.html. Instead login server (or web gate) is redirecting me to /login.do - which is dummy URL I created for authentication scheme. BTW, I do not have any other actions defined. It doesnt exist of course. Can somebody advise me on what I have missed, please?
