Hi Gurus,
I am not a DBA (am learning) but got called last night at midnight because my users always do that when they experience the problem through the application. It turned out that there was an instance failure due to some OS problems. Here are the logs from the trace file.

Fri Jun 02 00:13:40 2006
Errors in file t:\db\admin\bdump\db_lgwr_1392.trc:
ORA-00345: redo log write error block 25191 count 1
ORA-00312: online log 2 thread 1: 'U:\db\ORADATA\REDO.db.2A.LOG'
ORA-27070: skgfdisp: async read/write failed
OSD-04016: Error queuing an asynchronous I/O request.
O/S-Error: (OS 2) The system cannot find the file specified.
ORA-00345: redo log write error block 25191 count 1
ORA-00312: online log 2 thread 1: 'T:\db\ORADATA\REDO.db.2B.LOG'
ORA-27070: skgfdisp: async read/write failed
OSD-04016: Error queuing an asynchronous I/O request.
O/S-Error: (OS 2) The system cannot find the file specified.

The instance was not up until 8 am when the DBA came in, but it was shut down at midnight (we are running 24/7). Could you please tell me how this happened and what can be done to prevent this? Thanks a lot!

Ben
