Node Manager command 'COMMIT_CHANGELIST' failed unexpectedly

windows 2016; Oracle forms 12.2.1.3;

Installed successfully; Started Weblogic; started Nodemanager;

Connect through console; Forms are Up; Connect through EM push button start OHS got error:

Checking operation status on target /Domain_kbripms_domain/kbripms_domain/ohs1

Received error message from Node Manager Server: [Node Manager command 'COMMIT_CHANGELIST' failed unexpectedly due to:

[failed instances forms1 (create: already exists)].

can somebody help?
