Newbie - sar -d to mount points

I was wondering if someone could help me map the information from
sar -d 5 100
To the mount points on the filesystem.

e.g.

sar -d 5 100

10:40:41 device %busy avque r+w/s blks/s avwait avserv
sd13 18 0.2 21 194 0.0 8.9

I don't know this, but I want to say "Ohh that's /usr"
