need username and password for form builder 6i

i downloaded form builder 6i zip file. but what is the username, password and database name? iam unable to connect with my database. i tried with scott, tiger and my tns entry in database field and also with system/admin too.. i got this error.. "ORA 12154.TNS could not resolve service name". pls give me the username and password and also the database name..
