MRP0 - Background Media Recovery terminated with error 1119

Oracle DB 10.2.0.5
Primary RAC 2xnode - ASM - USING OMF - STANDBY FILE MANAGEMENT "AUTO"
Standby Single Instance - Unix File System - NOT USING OMF - STANDBY FILE MANAGEMENT "AUTO"
+ at log switch, upon addition of a NEW FILE at primary; below errors logged at standby alertlog and MRP stopped:
Media Recovery Log /opt/oracle/flash_recovery_area/xxx/archivelog/2011_08_07/o1_mf_1_86_73w9pznt_.arc
File #9 added to control file as 'UNNAMED00009'.
Originally created as:
'+DATA/orclz_xxx03/datafile/test.355.758508291'
Recovery was unable to create the file as:
'+DATA'
Errors with log /opt/oracle/flash_recovery_area/xxx/archivelog/2011_08_07/o1_mf_1_86_73w9pznt_.arc

ORA-01119: error in creating database file '+DATA'
ORA-17502: ksfdcre:4 Failed to create file +DATA
ORA-15001: diskgroup "DATA" does not exist or is not mounted
ORA-15077: could not locate ASM instance serving a required diskgroup
ORA-29701: unable to connect to Cluster Manager
Some recovered datafiles maybe left media fuzzy
Media recovery may continue but open resetlogs may fail

- Physical Standby created using SQL ... (No Broker configuration)
please address below queries in addition to providing your ideas to resolve the issue:
1. should i reset db_file_create_dest parameter at primary site i.e disable OMF?
2. what new-name should be given to that newly added file at standby file after setting standby_file_management to 'MANUAL'?
