Mismatched-identity error on launch from Cerner Code Console

When launching my application from the Cerner Code Console, I am getting the following error

error:

invalid_request

mismatched-identity

I'm able to launch the application fine standalone and have modified the scope for EHR launch to

launch patient/Patient.read online_access openid fhirUser profile

Any ideas on how to resolve?
