Menu in FORMS 6i

I created a Menu and attached the menu to my form. When the FORM appears the Menu items are at the top of the screen with the FORM in view. How do I make the Menu only appear when the FORM is launched. I want to choice the FORM from the Menu item. Also, I have a Oracle report that is a selection from the Menu choices. How do I execute the Oracle report from the Menu?
