menu icons not displaying after upgrade to 11g

Hello All,

I have this issue i am trying to resolve and i need help form you guyz.

i did upgrade my custom oracle forms 10gR2 applications to oracle fusion middleware 11gR2 successfully but some icons on the menu toolbar is not displaying. Please if anybody has faced such and was able to resolve it should please help me out.
