ksvcreate: Process(m000) creation failed

Hi Experts,
we are running 2 node RAC database 10gR2 (10.2.0.4). today suddenly on node2 we recieve above subject error in alert log. and after this error no one can make any session from node2. but interesting point is parameter

max_process = 1200
sessions = 1600

and at that time total number of sessions on both nodes are 500.

so is there any one who can tell me y this happened. we are running our database on AIX 6.1
