Interactive Report error - ORA-20001: get_dbms_sql_cursor error ORA-00904: : invalid i

Hi,
I am getting below error on IR. It is not giving that error consistently. I am not able to understand when that error occurs.
ORA-20001: get_dbms_sql_cursor error ORA-00904: : invalid identifier
I tried to look in the google but could not understand what exactly is the reason. I do not use mixed case columns in any of my tables and hence no need to use "double quotes" to enclose them. Not even for Tables. And none of the columns are of reserve words and I don't use any special characters other than underscore "_". I tried to look into DB trace files *.trc and alert log, but no mention about such error. The report works some time and some time not.
And yes, I am altering the tables by adding few new columns, but, I don't reference SELECT * in my procedure or pl/sql. Columns are called exclusively. None of the procedure / functions are in invalid state. Not sure, what's causing this issue. Can someone please help me to diagnose this. Thanks.
I use apex 4.2.6, oracle xe 11.2.0.2. Same issue observed even in apex 5.2
