Insert List of Numbers Into Table

I have a table MYTABLE which has 2 columns

country_code -- varchar2(3) short code for country

custom_no -- number(4)

Each country has a group of manua lcustom_no's assigned to them

e.g Germany 1000 - 1999

Netherlands 2000 - 2999

UK 3000 - 3999

How do I write an insert script to insert these values into the table

e.g

DE 1000

DE 1001

NL 2000

NL 2001

UK 3000
