INSERT and UPDATE transaction error

Hi all,

Currently we are facing some problem that is on INSERT and UPDATE transaction into database. Details as describe below
- First Transaction NOT inserted
- Last transaction has been updated TWICE (seems to replace the first one).

We are running on Oracle Form6i Patch 18 and Oracle 10g Database R2.
This problem only happen twice in a month and the program was running fine since last 5 years..
Pls help..

Note: The Update process was triggered by different User_ID and at Different Workstation
