Index Rebuild - based on height

Gurus,

I am preparing to rebuild some of my indexes online. My criteria would be:
Height>4
Deleted Rows>25%
storage used>25%

What is the best possible way to get a candidate list? Is there any example scripts from you guy's that I can start with?
