Icons in Forms 12.2.1.4

Hello,

I'm just wondering what needs to be done for icons to be enabled (for things like toolbars or LOVs) when you set up a fresh install of forms 12.2.1.4 and port over forms from another release. If anyone can point me to the relevant documentation or let me know what I've missed to get this working I'd be much obliged.
