How to write a Four button Alert

Good afternoon,
I'm using Forms 10g.

My program is looping thru a collection and based on a value in the collection I need to pop a question to the user.
This question has 4 possible answers: Yes, No, Yes to All and No to All.
If the answer to the question is Yes or No, I will be showing the question again (assuming the particular value in the collection exists).
If the answer to the question is the "to All" version, then I won't be displaying the question again.

I know that I cannot have an alert by itself, so I was thinking on using a modal window.

The problem is, how do I tell Forms that I'm waiting for a click on any of the buttons?
