How to visible or disable chech box

Dear all

I have form whare i save country_name data and i want set trigger,means when i execute query then it show only those checlboxes which have the country name 'Pakistan' otherwise do not show on form at run time plz tell me the way,how should i solve this problem.
my form fields are:
sp_Id
Country_Id
Country_Name
National(check box)
State(check box)
