How to setup oracle alerts in R12

Hi All,

I am trying to setup alert in R12. looks like its not allowing me to setup alert using Alert manager responsibility.

I did all the setups which is necessary for Alert Setup but when I clicked on verify button it prompted me error :APP-ALR-04124: Oracle Alert was unable to locate the INTO clause in SQL statement: &VALUE (ROUTINE=aluvss).

Can any one please help me get out of this situation and give me whole navigation for setting up alerts in R12.
