How to publish Smart on FHIR Application to Production and Cerner Market Place

Hi,

I have created and tested a SMART on the FHIR application with Cerner sandbox. I am looking forward to checkout steps/guide to publish that Application to Production and Cerner Market Place. Do we need to setup separate account or contact sales person or vendor to proceed on this? Any guide or link would be helpful
