How to insert date and time Date Dataype??

I am inserting with command in my Column "Time_Stamp" the datatype of this COlumn is "DATE"

TO_DATE('14-MAY-2012 08:00','DD-MON-YYYY HH24:MI')

Which is Fine .... But when i assign this COlumn from form i am getting error i am assinging like this

:time_stamp:=to_Char(sysdate,'dd/mm/yyyy hh:ss:mi');

then i got this error

is it Database Issue?
