How to insert a node using DOM insertBefore ?

Hello,

Please help me to insert an element using DOM.
I tried using insertBefore(). But it says
"HIERARCHY_REQUEST_ERR: An attempt was made to insert a node where it is not permitted"

How do i fix it ?
