How to get approved 'Entitlement Name' with 'Request ID' details?

Hi Experts,

We are trying to get Beneficiary_Name,Request_Id,Request_Creator,Request_Creation_Date,Request_Approver, Request_Result, and Entitlement_Name values from OIM11gR1 database(back-end). We got all information except 'Entitlement(role) Name’. Any one can you help on this.

Here we are talking about EBS Entitlements(Roles).
