How to fix a ODA VM booting and stuck at device-mapper: ioctl

I built a new VM on our Producation ODA X4-2 system.  It is at version 12.1.2.0 of the ODA_BASE.  Don't know what to do to get VM up again.  Any suggests appreciated.

I finished installing the /u01 and swap disk.  To make sure the disk show up after reboot, we executed shutdown -r - command as root.

now the VM console is stuck at the following:
