How to find Enable_list_lamp LOV ?

Hi I want to show calendar for one of the date items so I can input date directly. In the earlier articles it wrote"Set the LOV of the item for which calendar to be attached as ENABLE_LIST_LAMP LOV, which is included in the TEMPLATE form. "

Just want to know where is this TEMPLATE form because I cant see the ENABLE_LIST_LAMP LOV . Thanks !
