How to create comments box in RESPOND type Notification?

Hi,

I am new to Workflows
I have created a custom workflow which needs Approve/Reject Notification.
I want to add a field for the Approver to supply Approval or Rejection commands.

My questions are
1. How to create a text area for comments or notes?
2. How to capture the user comments from the notification?
