How can I stop upper/lower case table & column names changing?

I have been through the internet and applied all the suggestions on changing the preference to prevent my table name and column names from defaulting to upper case letters when I insert a new table. This just recently started occurring and I did not change the preference prior to this happening. I have tried editing the Oracle Formatting, unchecking the generate case sensitive SQL Import Methods, and changing the Completion Insight.

This behavior is not allowing me to update existing tables with new data because the table is not recognized because of the upper case of the table not matching the case of my source document.
