How can I create an interactive form based on an RTF template in APEX 5.0

Due to budget issues, what other options can I use besides the Bi Publisher plug-in with APEX to output data from an .rtf template?  Keep in mind the final output will need to be .pdf.

Things I have tried

- Shared Components >> Report Queries and Report Layouts.  My report query runs fine when tested, but if I try and upload a xsl:fo template and then output the form into a .pdf, it cannot decode the form. 

I am trying an HTML based form, but having difficulty setting this up.  Does anyone have detailed instructions for setting up something similar to what I am describing here?

The report I am wanting to build must interact with the user before outputting.

Here are the procedures that an end-user must take when logging into APEX.

1. The end-user will select an interactive or similar report to run "My report"

2. The end-user will provide a number i.e. 548965

3. When submitted, the report will generate an interactive report (see attached report) and provide the details based off the number from step 2 i.e. business name, street, city, state, etc...

4. The user must be able to manipulate certain fields of the report form such as check boxes and other drop-down menus.

5. The report will then be outputted into a .pdf form once all editions have been made
