how can add nested pivot columns in header?

i have using the following threat for pivot table

pivot table with year and decode text

i could not add nested header and at the end total,grand_total columns with calculation(sum,multiple).

i expecting like this...

development platform: Oracle Database 11g Enterprise Edition 11.2.0.4.0 64bit Production
