hosted app still has "Malformed argument to wwv_flow_custom_auth_std.login"

Referring to thread 1737055

It looks like this problem was supposedly a result of the ApEx 3.0 upgrade, but I'm still having this problem, even though it was purportedly fixed.

My problem is a double-whammy: somehow, my account got locked, so I can't even get into developer mode to further diagnose what's going on.
