HFM - Smart view - Invalid dimension ID when refresh of HFM forms thru SV

I am creating provisioning for end users. I am having 2 issues:

1. when I log into smart view using the test ID I created, it says "you do not have the proper access rights to perform this task". However when I log in first using some admin ID and then disconnect and after that use the same test ID, I am able to connect. Then I get the problem specified in 2 below

2 The end users are required to take HFM forms in Smart view and submit. When I log in using the test ID I created I am able to take the form to Smart view. But when I try to refresh the form I get the error message *"Invalid dimension ID"*. Also it does not allow me to submit the form data.

I have provisioned the user ID with the following for HFM:
Default
Reviewere 1
Load Excel data
Data form write back from Excel

Not sure if I am missing something.
