HFM Icons Issue in Workspace 11.1.2.3.500

I have an issue in Workspace where the HFM application via Navigate > Administer > HFM App is not showing the usual icon. Rather it look like a broken icon.

Upon opening the application, receiving the following error. 'hfmadf.application'

In the HFMWeblog0, I do see the following error.

I've stopped the services, re-deployed HFM web/app as well as web server (IIS) on Foundation, started services back up, but issue still persists.

Has anyone seen this error before in the HFMWebLog and can offer some assistance where to look next?
