FRM-92050: Failed to connect to the server :/forms90/i90servlet:-1

I have just installed Oracle 9i Development Suite Release 2 (9.0.2.0.0) with no errors.

I have launched the forms builder and created a basic form on the all_tables table to test that I can build and run a form in this environment.

When I run the form I get the error FRM-92050: Failed to connect to the server :/forms90/i90servlet:-1

I am guessing that this is a configuration issue as everything is at the default values, set during the install.

I am running Windows NT 4.0 patch 6a with 128 bit encryption and using Internet Explorer 6.0.2600 also with 128 bit encryption.

Can anyone shed some light on what changes I need to make to enable this form, or any other, to run.
