FRM-40654 record updated by another user. Requery to see change.

We upgraded from 9i to a 10g environment (DB 10.2.0.3, AS 10.1.2.2, Developer Suite 10.1.2.2) in November. A few days after the upgrade and ever since, the users (there are 2) have been reporting FRM-40654: record updated by another user. requery to see change. It is very sporadic. It may happen 2 or 3 times in one day and then not for a few days. This error did not occur in 9i. Also, when they receive this error, they acknowledge the error in the dialog box, but they cannot requery or do anything. Anywhere they click in the form causes the error dialog box to popup. They end up closing the browser and restarting their session.
I have researched Metalink and tried the suggestions (set the primary keys in the block, enforce primary key = yes, etc) ...to no avail. There are no db triggers or form triggers that update the record. The data block is based on db table.
I'm not sure what to try next. Any ideas?
