FRM-40654 Record Has Been Updated. Requery To See Change

Hi Gurus,
I have been facing this error when a inserted two records in child block by populating a master item through a LOV.
When i tried to insert 3rd time for another master item then just at the time of populating it from LOV this erro is coming always.

FRM-40654 Record Has Been Updated. Requery To See Change

Please help me on this.
