FRM-40301 Query caused no records to be retrieved

I have a simple form, and i can (query) and view the records if i input data through the form. But when i insert data from the backend (say TOAD), i am not able to view that record from the form. It ends up with
FRM-40301 Query caused no records to be retrieved error.

Is there any kind of setting that has to be turned on or off in forms?
