Fragmentation and how to avoid this

Hi, I am using 11g oracle EE. I have tables and with fragmented.

1. how to see latest fragmentation on indexes and tables.?

2. how reduce values in num_row , EMPTY_BLOCKS AVG_SPACE CHAIN_CNT in dba_tables

3. I run dbms_stats_gather_table_stats and still that row_num, chain_cnt remain same . why and how to reduce this.
