Forms Error FRM-40654 - Question ?

Hello,
One of my users is having a strange error, when he is entering data on a screen and trying to save it. He is getting a message FRM-40654: Record has been updated by another user: Re-query to see change.

The users claims that he is the only user and also says that he is 99.9% confident that he was the only one as he was creating it and had not filled in all the fields fully or submitted it so no one else would know it was there.

What could be the reason for this error ? Please advise
