Forms EAR and JInitiator 1.3.1.5

Where can I get the Forms EAR for OC4J? Similarly, where can I get Jinitiator 1.3.1.5? The Forms 6i Patch 9 documentation mentions both but I can't find either one in the downloaded patch?

Also, are the Forms Servlet/Forms Listener Servlet certified to run on OC4J with Patch 9? Is Reports CGI + OC4J a certified combination? For somebody that needs just forms and reports, the full iAS 9i EE installation is way overkill. I'd rather just have iAS9i Core + Forms + Reports.
