Forms Builder crashes when attempting to run forms locally (ntdll.dll)

Whenever I attempt to run the debugger or a form locally in Oracle Forms Builder, the application crashes right after the form is compiled.

The steps used to recreate the problem:
01. Load a form in Oracle Forms Builder
02. Connect to a database
03. Compile the form
04. Access the debugger.

System settings:
Windows XP Pro with Sp3
Internet Explorer 7
JInitiator 1.3.1.13
Oracle Forms 9.2.0.4
Oracle 9i DB

I have done the following as troubleshooting methods however have been unsuccessful in getting the debugger to run.

01. Deinstalled Oracle DS 9.2.0.4 and Oracle 9i.
02. Ran C cleaner.
03. After reboot, ran C cleaner again (more problems were found).
04. Reinstalled and ran Spybot (no problems were found).
05. Rebooted.
06. Reinstalled Oracle DB 9i
07. Reinstalled Oracle DS 10g 9.2. and patch set 9.2.0.4
08. Reinstalled Oracle Documentation Suite
09. Reset Internet Explorer settings to default.
10. Reset Firefox settings to default.
11. Disabled all plugins, applications related to IE except for JInitiator 1.3.1.13
12. Set IE as the default browser.
13. Uninstalled all versions of Java VMs
14. Uninstalled all versions of JInitiator.
15. Reinstalled JInitiator 1.3.1.13
16. Cleared the JAR Cache
17. Installed Windows XP SP3.
18. In safe mode, copied the file ntdll.dll from another user's machine to both the \windows\system and \windows\system32 folders.

Whenever doing a google search for ifbld90 or ntdll.dll, I have been unable to find any results pertaining to the issue I am having. The same goes for metalink.
