Forms 12c - User Authentication issue

Dear All,

We have upgraded Oracle forms from 11g to 12c.

Database was also upgraded from 11g to 12c.

After logging with a user (After sometime) receiving the below message.

Note: frmweb.cfg, a userId hard-coded and that user does not get this issue when accessing forms.

Is there any parameter need to be added in config?

What causing the issue?

How to troubleshoot and resolve this issue?

Any inputs is much appreciated, If you need any info kindly let me know.
