Forms 10g runtime error

Hi Folks,

I am trying to run a form that was developed from ground up. The data block is based on a custom table and designed using a Data Wizard. First time around it asked for JInitiator and then I had it installed (from C:\DevSuiteHome\jinit\jinit.exe)by accepting defaults. All this is on my local machine. When I try to run the form (from Form Builder after starting the OC4J instance) I encounter the following error. the URL generated is http://localhost:8889/forms/frmservlet

Internet Explorer has stopped working.

Looks like some configuration may have gone wrong. Any suggestions or thoughts to resolve the issue is very welcome.
