Form layout design examples

Does anyone have any suggestions on where on the web I might find examples of various Forms layouts? I'm having a hard time "thinking outside the box" when it comes to a pleasing and efficient layout that will allow the forms to fit on various monitor sizes as well as make logical sense to the user. I think what I have now is OK, but I'm sure it could be better.
