Form closes abruptly when FORM_TRIGGER_FAILURE and Exit_Form is called

Hi,

we are using forms 11g. There is issue that whenever control reaches FORM_TRIGGER_FAILURE or Exit_Form commands that form is closed abruptly at the same moment. It gives some error message to contact administrator.

Pls help to resolve this issue.
