Form Builder stopped working correctly

I had a working Oracle setup on home computer, a PC running Windows 7 Home Premium

    

     12c Database

     Fusion Middleware

     Forms

     Reports

     Apex

Had it all installed, configured, and running correctly.

Some 8 months have passed and I cranked it all up to test some code. Some passwords had expired, preventing me from logging into Apex Administration Services, but I reset the Admin account password and fixed that.

When I try to run a form in my browser i just get a quick flash of "Oracle Forms" text and then a blank page.

When I use Firefox, the URL is http://mysystem..attlocal.net:9001/forms/frmservlet  but isn't the form appended to the end of that URL?

Shouldn't it be in the form of :http://example.com:8888/forms/frmservlet?config=my_application?

Why isn't my form.fmb text being appended to the URL after the frmservlet text?

It is even worse when I run the same form in Internet Explorer

now my URL doesn't even have the correct Forms Server listener port (9001) and contains a bunch of random text :  http://localhost:9330/mxTLyQt0GjrlRZ0LF8im4zvRu9vGWoIqvdjJc1TRWpUmU5QM

What happened to my Form Builder setup? I haven't changed or updated anything. I just went several months without running anything Oracle.
