form 12c problem in execution

Hi all

i am facing problem in running oracle form which on execution displays error page 404 not found. Here are my settings;

Windows Server 2012 R2 (64 Bit)

Forms [64 Bit] Version 12.2.1.2.0 (Production)

Oracle Database 12c Enterprise Edition Release 12.2.0.1.0 - 64bit Production

Java Version: Oracle Corporation, 1.8.0_101

OC4J : Oracle Containers for J2EE 10g (10.1.3.5.0)

Any Help is highly appreciated...
