Form 11G not responding when I try to run the form. Gurus Please advice

I recently installed Forms 11 g 64 bit version on my windows 10 laptop . I was able to creat form but when i hit run I see nothing in the browser... can some one please help

the sequence of steps that i followed are:

Insured that database was up and running ( got proper response from database queries)

Invoke node manager

started weblogic server

invoked the forms server and created a test form using connection to database THE ISSUE IS WHEN I TRY RUNNING THE FORM I GET

HERE IS THE VERSION:

SO I SEE THE FOLLOWING BLANK with the url as http://desktop-at0a1vl:8888/forms/frmservlet
