Flash Pie Chart - XML Loading Faiiled

I am getting an XML Loading Failed error on my production server.

The URL is http://minipro.panynj.gov/pls/apexcfprd8/p=120:1

I get the following error:
XML Loading Error
http://minipro.panynj.gov/pls/apexcfprd8/apex_util.flash?p=120:1:3476468973004364:FLOW_FLASH_CHART_R132860583

I didn't find anything in the error_log file pertaining to this error.

I found these entries in the access_log file:

Can someone please help ASAP? It is happening on the production server.
