Excel download missing csv file extension

I have seen lots of probs reported about the excel download - but can't find this one mentioned.
We've built a great little reporting system that works really well except one small area. On most of the pages the users can happily download a report to excel after we have enabled the csv export option. When it works, the window that pops up will say something like filename= CAODFGH.csv and do you want to open or save it?
On other pages though, when they click on the link it brings up the same popup, but the filename listed does not have an extension, so when you go to open it, windows brings up the popup to choose an application, and when excel is chosen it just pastes the raw csv output into column 1.
It perhaps looks like it occurs when the report is generated inside a page that uses multiple columns - though this is not always the case?
The problem occurred in v1.5 and we have just upgraded to v1.6 and the problem persists.
